The following is Mazin Al-Khafaji's user guide to the bespoke products for the most common conditions.
Psoriasis
Bai Bi Gao (Ointment) - 30ml, 50ml
Number One Ointment for psoriasis. All types of psoriasis. Best results are achieved when the patient applies a layer of the ointment and then occludes the area overnight with Clingfilm (this increases efficacy substantially, so important to encourage the patient to do so). Usually I prescribe this to be used at night with Clingfilm, and either Bai Bi Shui Gao (the cream), Lei Gong Teng Shui Gao or Bai Bi Er Hao Shui Gao for the day. It is messy (& staining) to use, but really worth the effort in most cases. Should see change in 2 to 3 weeks if not sooner when the patient uses it correctly every night.
Use with care with very active psoriasis (psoriasis that is on the way to becoming erythrodermic) as with all topical measures (best to use blander creams like Bai Bi Er Hao Gao).
In a very small proportion of patients (especially when used with Clingfilm when the psoriasis is very active), the skin becomes sore and red. If this happens then discontinue use immediately and apply Bai Bi Er Hao Shui Gao until settled, then try using Bai Bi Shui Gao, or even Bai Bi Shui.
Bai Bi Shui Gao (Cream) - 30ml, 50ml
As for Bai Bi Gao (i.e. for all types of psoriasis), but is a cream therefore easier to use in the day (no need for occluding with Clingfilm). Often used in conjunction with Bai Bi Gao (i.e. use Bai Bi Gao with occlusion at night, and Bai Bi Shui Gao without occlusion in the day). If the colour is unacceptable use Lei Gong Teng Shui Gao or Bai Bi Er Hao Shui Gao.
Bai Bi Shui (Alcohol tincture) - 50ml, 100ml
For any psoriasis on face or scalp (where ointments and creams are cosmetically unacceptable), although some patients are still able to use Bai Bi Gao (with or without occlusion) & Bai Bi Shui Gao on the face and scalp in the evenings.
Many patients like to use it on the body & limbs as well since it is not messy like Bai Bi Gao. Good at subduing itching.
It can cause dry skin if used excessively (since it is alcohol based) so I also prescribe it with Bai Bi Er Hao Gao or Zi Cao Shui Gao.
Bai Bi Er Hao Shui Gao (Cream) - 30ml, 50ml
Good for very dry fissured or very active psoriasis (where bland creams are needed), or as secondary cream with all types of psoriasis to moisten and hydrate the skin. Often used during the day since it rubs in without leaving a colour (see Bai Bi Gao).
Run Fa Jing You (Oil) - 100ml
For psoriasis, eczema, seborrhoeic dermatitis (dandruff) of the scalp. Dry itchy scalp. Apply to scalp, then wrap with cling film or shower cap overnight. Wash off with mild shampoo. Depending on severity of condition apply once a day or once every several days.
Run Zu Shui Gao (Cream) - 50ml, 200ml
Strong emollient cream for moistening, nourishing and thinning hyperkeratotic skin. Use for palmer planter hyperkeratotic eczema & psoriasis.
Yu Huang Gao (Ointment) - 30ml, 50ml
All types of psoriasis. Best results are achieved when the patient applies a layer and then occludes the area overnight with Clingfilm (this increases efficacy substantially, so important to encourage the patient to do so.
Usually I prescribe this to be used at night with Clingfilm.
Eczema
Zi Cao Shui Gao (Moisturising and healing cream) - 50ml, 200ml
Number one emollient cream moistening all dry eczemas but also has very good cooling blood properties. So is an excellent emollient as well as moisturiser and anti-inflammatory. Use in conjunction with Hu Zhang Shui Gao for dry eczema, or Shi Zhen Shui Gao to moisten and hydrate the skin. Use 3-5 times a day instead of other moisturiser/emollients.
Huang Lian Shui Gao (Cream) - 30ml, 50ml
Use for dry atopic eczema. Infantile, childhood or adult. Either localized or with widespread erythema with profuse scaling (subsequent to inflammation), and severe itching. One disadvantage is the yellow colour it leaves on the skin so not cosmetically suitable for the face.
I always prescribe it with Zi Cao Shui Gao.
Huang Lian Gao (Ointment) - 30ml, 50ml
Use for dry atopic eczema. Infantile, childhood or adult. Either localized or with widespread erythema with profuse scaling (subsequent to much inflammation) and severe itching. Also used for hot Lichen Simplex.
The only problem is the oily nature of the ointment. In practice some patients find it really good, others don’t...which is very typical with external medicine.
I always prescribe it with Zi Cao Shui Gao.
Shi Zhen Shui Gao (Cream) - 30ml, 50ml
For all eczemas. Can be a little drying, which makes it most appropriate for slightly wet eczemas, or well-circumscribed oedematous lesions.
I always prescribe it with Zi Cao Shui Gao to ameliorate the drying effect.
Shi Zhen Gao (Ointment) - 30ml , 50ml
As for Shi Zhen Shui Gao, but an ointment. Has stronger action than Shi Zhen Shui Gao as has more active ingredient (30% as apposed to 10%). Best to use for slightly moist or oedematous lesions, or when the skin is prone to infection.
I always prescribe it with Zi Cao Shui Gao to ameliorate the drying effect.
Zi Cao Run Chun Gao (Lip salve)
For dry, fissured lips. Angular chelitis, exfoliative cheilitis. Contains very similar ingredients to ZI Cao Shui Gao which makes it ideal for dry fissures inflamed skin.
Rich Vitamin E Cream (Cream) - 30ml, 50ml, 200ml
A simple pure cream, with 10% by weight of vitamin E and no fragrance. Occasionally people with eczema may react badly to Zi Cao Shui Gao so this is prescribed instead as an emollient for dry parched and under nourished skin.
Also used as an effective face and neck cream to moisturise and nourish.
San Bai San (Powder) - Sachet
Powder for acute phase of pompholyx eczema. Pour boiling water on it, and then stir to dissolve. Once dissolved hold the hands or feet over the steam, this seems counter intuitive but has a good effect to alleviate itching quickly. When the liquid is cool enough place hands or feet in to it, and soak for 15 minutes. Very effective at subduing itch from acute pompholyx.
I always prescribe it with Zi Cao Shui Gao to ameliorate the drying effect as soon as the vesicles have dried.
Run Fa Jing You (Oil) - 100ml
For eczema, seborrhoeic dermatitis (dandruff) & psoriasis of the scalp. Dry itchy scalp. Apply to scalp, then wrap with cling film or shower cap overnight. Wash off with mild shampoo. Depending on severity of condition apply once a day or once every several days.
Gan Cao Shui Gao (cream) - 50ml, 200ml

Run Zu Shui Gao (cream) - 50ml, 200ml
Strong emollient cream for moistening, nourishing and thinning hyperkeratotic skin. Use for palmer planter hyperkeratotic eczema & psoriasis.
Acne
Dian Dao Shui Gao (Cream) - 15ml
Acne & rosacea. All stages. Usually apply in the evening or night, leave it on till morning. A little yellow and messy (therefore usually only used at night), but effective at reducing erythema and pustules quickly.
Usually used together with Fei Zi shui (for daytime).
Fei Zi Dian Dao Shui (Gel) - 15ml
I prescribe this for acne (and pustular rosacea) to apply at night. Similar to Dian Dao Shui Gao, but being a gel it’s easier to apply.
Usually used together with Fei Zi Shui (for daytime).
Fei Zi Shui (Gel) - 15ml
I prescribe this for acne (and pustular rosacea) to apply during the day when Fei Zi Dian Dao Shui or Dian Dao Shui Gao is not possible because of yellow colour.
Hu Zhang Shui Gao (Cream) - 30ml, 50ml
For cooling dry, red atopic eczema, and also for moistening and hydrating. Contains Bo He Nao so best to use with caution on babies and on lesions around the eyes.
I always prescribe it with Zi Cao Shui Gao
Rosacea
Jiu Zha Shui Gao (Cream) - 30ml
Especially for telangiectasia and erythema. Use in conjunction with Bai Bu Shui (apply Bai Bi Shui to the area first and after it dries, apply Jiu Zha Shui Gao) or Dian Dao Shui Gao (when pustular lesions are pronounced).

Other
Bu Gu Zhi Shui (Alcohol tincture) - 50ml, 100ml
For vitiligo. Also effective for alopecia areata. Use with extreme care or not at all in direct sunlight. Patients must monitor themselves very carefully when they use it, initially exposing their skin to no more than 1-2 minutes in direct sunlight to gauge their response (even less in Summer). With experience can have longer exposure.
Expect to see pinking of area before repigmentation (typically staring from follicles).
Wu Mei Shui (Alcohol tincture) - 50ml, 100ml
For vitiligo (although works differently from Bu Gui Zhi Shui Gao, this, without the need of UV light). Less strong than Bu Gu Zhi Shui, so I prescribe it when the sun is strong (although still wise to be careful in direct sunlight, and exercise same caution as for Bu Gu Zhi Shui until the patient gets the measure of it).
Apply with finger twice a day and allow to dry naturally.
Dian Dao Xi Ji (Wash) - 100ml
For the bacterial infection Erythrasma. Apply with finger twice a day and allow to dry naturally.

Ce Bai Shui (Alcohol tincture) - 50ml, 100ml
Alopecia areata. Apply with finger 2-3 times a day and allow to dry naturally.
Cang Er Zi You (Nose Drops)

Dian Dao Xi Ji (Wash) - 15ml
For the bacterial infection Erythrasma.
Hei Bu Yao Gao (Ointment) - 60g
Keloid scars. Apply once every 24 hours. Wash off before second application. This is a thick tarry ointment that needs to be kept in a glass jar. The glass jar should be placed in a hot water bath for a short while to make it less viscous in order that it can be applied easily. Once it dries it forms a black skin that can be washed off with water after 24-48 hours.
Cu Pao Fang (Vinegar tincture) - 500ml
For fungal disease of the feet and hands. Soak affected part for 15 minutes once or twice a day. Discard after 2 weeks of use. Use for dry forms of the disease. It should not be used when the skin is broken, as the vinegar will sting (use a water soak rather).
Ti Xuan Ding (Tincture) - 50ml, 100ml
Fungal disease of the body. Apply first, allow to dry naturally, then apply the cream Ti Xuan Shui Gao. Use 2-3 times a day.
Ti Xuan Gao (Ointment) - 30ml, 50ml
Fungal disease of the body. Usually used in conjunction with the tincture Ti Xuan Ding. Use 2-3 times a day.

Xue Jie Gao (Ointment) -30ml, 50ml
For Haemorrhoids. Apply 1-2 times a day.
Be aware that all ointments applied to the anal area may cause autosensitisation. Discontinue use if the area becomes inflamed.
Ya Dan Zi You (Oil) - 10ml
Apply to common warts. Occlude with strong tape (Duct tape is ideal) after applying the oil. After 24 hours wash off remaining oil, scrub off any loose skin, and reapply oil and repeat the process daily until the wart clears. It’s very effective for common warts especially if used with internal medicine (can take several weeks to clear).
Try and avoid contact with unaffected skin.
